• If you have out-of-network physical therapy benefits, you may be able to submit a superbill (receipt) to your insurance company for reimbursement.
• I do NOT participate with Medicare or any Medicare Advantage plan, therefore I can only provide dry needling & wellness services for Medicare beneficiaries. Please contact UCHEALTH or Select Physical Therapy for physical therapy.
